      目的观察射频热疗联合介入持续动脉化疗治疗21例晚期大肠癌患者的临床初步疗效。 方法对21例局部复发和伴有肝、肺、骨、腹腔、盆腔及腹股沟淋巴结转移的晚期大肠癌患者经射频热疗联合供养靶动脉介入灌注化疗。每名患者至少进行2个周期的介入化疗联合热疗。化疗方案为第1~3天CF 200 mg/m2,静脉滴注;第1天DDP 80 mg/m2,第1~3天如肾功能改变则改用Vp-16 60 mg/m2,动脉滴注;5-FU 2 500 mg/m2,持续动脉滴注72 h。肝转移血供丰富者,先将ADM 30 mg/m2和MMC 6 mg/m2加入超液化碘油10~30 ml中进行肝动脉灌注栓塞。热疗频率40.68 MHz,电容式加热,电极直径20~25 cm,入射功率500~800 W,反射功率20~40 W。插管化疗第2天开始进行热疗,每72 h 1次 ,每次50~70 min,每疗程化疗配合4~8次热疗。 结果病灶近期疗效以CT或B超检查结果作为评价标准,CR 0例, PR 14例,总有效率为66.67%。患者化疗未出现严重的不良反应。热疗不良反应为局部疼痛和脂肪硬结,不需特殊处理。 结论射频热疗联合介入持续动脉化疗是治疗晚期大肠癌的安全、有效的治疗方法,能提高治疗效果和改善生活质量。

      Objective To observe the therapeutic efficacy of continuous arterial infusion chemotherapy combined with radiofrequency hyperthermia in the treatment of patients with advanced colorectal cancer. MethodsA total of 21 advanced colorectal cancer patients with various distant metastasis were treated by the combined continuous intra-arterial infusion chemotherapy and radiofrequency hyperthermia. For the chemotherapy, a dose of 200mg/m2 surface body area of calcium folinate (CF) was given for 1 to 3 days. 80mg/m2 of cisplation was infused intravenously for the first day and replaced by etoposide Vp-16, 60mg/m2 for 1~3days in patients with renal dysfunction. For intra-arterial infusion, a dose of 500mg/m2 5-FU 2 was given for 72 hours. For patients with liver metastases, chemoembolization(ADM 30mg/m2+MMC 6mg/m2 mixed with ultra-liquefied lipiodol) was carried out. Radiofrequency hyperthermia with a frequency of 41MHz was performed on the second day after chemotherapy. ResultsResponse rates were assessed by CT scan and ultrasonography. The overall response rate [CR+PR] of the cases was 66.67%. No serious side effect or complication was found in the course of chemotherapy. Local pain and lipid nodule were occasionally observed in some patients after hyperthermia. ConclusionContinuous intra-arterial infusion chemotherapy in combination with radiofrequency hyperthermia is an useful and safe method for the treatment of patients with advanced colorectal cancer.
