      目的观察热矿泥浴对大鼠佐剂诱导性关节炎(AIA)的治疗效果及作用机制。 方法Wistar大鼠按AIA造模后,随机分为3组,分别给予热矿泥浴、普通黄泥浴共19 d,空白对照组则不予处理。连续观察关节炎症积分,第30天取标本,检测血清肿瘤坏死因子(TNF-α)含量,观察关节的组织病理学改变,原位杂交法观察基质金属蛋白酶-1(MMP-1)和细胞间粘附分子(ICAM-1)mRNA在关节滑膜和软骨中的表达。 结果热矿泥浴能显著降低AIA的关节炎症的程度,包括炎症积分、组织切片的滑膜细胞增生和软骨侵袭。热矿泥浴还能显著降低血液中的TNF-α含量,抑制滑膜和软骨细胞的MMP-1 mRNA以及软骨细胞ICAM-1 mRNA的表达。普通黄泥浴也能抑制软骨细胞的ICAM-1 mRNA的表达。 结论热矿泥浴能显著抑制大鼠AIA的关节炎症,其机制与抑制TNF-α的产生以及MMP-1和ICAM-1在滑膜和软骨中的表达有关,且这种作用不单纯是泥浴的物理作用,可能主要是热矿泥浴的化学和生物学作用所致。

      Objective To observe the effect of thermal mineral mud-bath(TMM) on the adjuvant-induced arthritis(AIA) of rats and study the therapeutic mechanism of TMM. MethodsThirty-nine Wistar rats with AIA were randomly divided into three groups: a TMM group, a loess mud-bath (LM) group and a control group, the mud-bath therapy lasted for 19 days, and no intervention was given to the control group. The arthritic scores were assessed once every other day for 30 days. The serum tumor necrosis factor-α(TNF-α) was measured by ELISA, the histopathologic change of the inflamed ankle were observed, expression of matrix metalloproteinase 1(MMP-1) mRNA and intercellular adhesion molecule 1(ICAM-1) mRNA in the synovium and cartilage were detected by using the in situ hybridization. ResultsTMM alleviated arthritis significantly. Besides, TMM also decreased the amount of serum TNF-α, suppressed the expressions of MMP-1 mRNA in synovium and cartilage and ICAM-1 mRNA in cartilage significantly. Similarly, LM suppressed the expression of ICAM-1 mRNA in cartilage significantly. ConclusionTMM is able to suppress the arthritis in rats with AIA, and the mechanism may be attributed to the suppression of the serum TNF-α, and the expression of the MMP-1 mRNA as well as ICAM-1 mRNA in the synovium and cartilage. These effects are not only caused by the physical, but also by the chemical and biologic actions of the thermal mineral mud.
