      目的探讨针刺对急性脑梗死的治疗作用及其机制。 方法将32例急性脑梗死患者随机分为针刺组和对照组,每组各16例。针刺组接受针刺(每周5次,共3周)和常规治疗(静脉滴注丹参及对症支持治疗等),对照组仅接受常规治疗。两组患者均在治疗前、后采用改良爱丁堡-斯堪的维亚脑卒中评定量表(MESSS)评估神经功能,检测磁刺激运动诱发电位(MEP)、短潜伏期体感诱发电位(SLSEP),并测定血清神经元特异性烯醇化酶(NSE)和髓鞘碱性蛋白(MBP)含量。 结果治疗3周后,针刺组患者MESSS评分、血清NSE及MBP含量明显降低,且MEP中枢运动传导时间(CMCT)、SLSEP中枢感觉传导时间(N13~20IPL)明显缩短,与对照组比较差异均有显著性意义(P<0.05)。 结论针刺能促进急性脑梗死患者神经功能的康复,其机制可能与其保护神经元受损、调节中枢运动及感觉传导通路的兴奋性有关。

      Objective To explore the therapeutic effects and the mechanism of acupuncture on patients with acute cerebral infarction. MethodsThirty-two patients with acute cerebral infarction were recruited and randomly divided into an acupuncture group (group 1) and a control group (group 2). Acupuncture treatment (5 times a week, for 3 weeks) was conducted solely in patients of group 1, in addition to the routine treatment (including intravenous drop of Dan Sen injection and supportive treatment) for both groups. The neural functions of all patients were assessed by use of improved MESSS assessment, while magnetic stimulation motor evoked potential (MEP),short latency somatosensory evoked potential (SLSEP), and the concentrations of serum neuron-specific enolase (NSE) and myelinbasic protein (MBP) were measured before and after treatment. ResultsCompared with group 2, MESSS, concentrations of serum NSE and MBP in group 1 decreased dramatically, and central motor conduction time (CMCT), N13~20 interpeak latency (IPL) shortened markedly 3 weeks after treatment. The difference between the two groups was significant (P<0.05). Conclusion Acupuncture could promote the recovery of neural function of patients with acute cerebral infarction, and the mechanism might be related to the protection of the neuron and regulation of the excitability of central sensory and motor pathway.
