      目的 探讨吞咽专科培训对治疗师岗位胜任力的影响,并分析其影响因素,为优化培训体系提供依据。 方法 采用前瞻性队列研究设计,对2018年至2024年参加中国康复医学会吞咽专科培训课程的264名治疗师进行问卷调查。采用自制的一般资料调查表和信效度良好的吞咽专科治疗师核心能力自评量表收集数据,使用配对样本t检验比较治疗师培训前后的量表得分差异,用独立样本t检验或单因素方差分析探讨其影响因素。 结果 本研究累计发放问卷264份,共回收258份,回收率为97.73%。核查后,剔除无效问卷11份,最终获得有效问卷247份,有效回收率为93.56%。培训后,治疗师核心能力自评量表各维度得分均有所提高,总分由(45.11±8.53)分显著提升至(62.09±8.94)分(P<0.05)。单因素方差分析结果显示,年龄>30岁、副高及以上职称、所在科室为康复科、所在单位设置有吞咽专科治疗师岗位及制订有吞咽专科治疗师管理制度的治疗师,培训后的核心能力自评量表得分较高(P<0.05);学历、医院级别、工作年限及是否获得吞咽障碍专项继续教育对核心能力自评量表得分无显著影响(P<0.05)。 结论 吞咽专科培训能显著提升治疗师的岗位胜任力,其培训效果的长期维持不仅与个人资历有关,还有赖于医疗机构提供的组织支持,如设立明确的专职岗位和规范的管理制度。

      Objective To explore the impact of specialized dysphagia training on the competence of therapists, and to analyze the factors most influencing it to provide a basis for optimizing their training. Methods This was a prospective cohort study involving 264 therapists who completed the specialized dysphagia training program of the Chinese Association of Rehabilitation Medicine and obtained qualification between 2018 and 2024. They were surveyed using a self-designed general condition questionnaire and the validated Core Competency Self-assessment Scale for Dysphagia Specialized Therapists. Paired-sample t-tests were used to compare the competency scores before and after training, and independent-sample t-tests or one-way ANOVA was employed to isolate the most influential factors. Results All dimensions of the therapists′ core competencies had improved significantly after the training. Univariate analyses indicated that age, professional title, department, the existence of dedicated dysphagia therapist positions in an institution and the institution′s management system were all useful predictors of post-training competency scores. Conclusions Specialized dysphagia training can significantly enhance the core competencies of therapists. However, its long-term effect relies not only on individual seniority, but also on organizational support from healthcare institutions, including the establishment of specialized posts and standardized management systems.
