      目的 探究电针八髎、关元、中极穴联合经颅磁刺激对脊髓损伤后神经源性下尿路功能障碍(NLUTD)患者膀胱功能和总体预后的影响。 方法 选择NLUTD患者96例,按随机数字表法分为磁疗组和联合组,每组患者48例。磁疗组给予常规治疗和经颅磁刺激,联合组在磁疗组基础上给予电针八髎、关元、中极穴治疗。于治疗前和治疗8周后(治疗后)记录2组患者排尿情况(导尿次数和平均尿量),同时行尿流动力学(残余尿量和膀胱最大压力)检测,测量其最大肾盂分离宽度,并进行国际下尿路症状(LUTS)评分和世界卫生组织生活质量测定量表(WHOQOL-BREF)评估。于治疗后对2组患者进行疗效评价。 结果 治疗后,2组患者的膀胱最大压力、日平均排尿量均显著高于组内治疗前(P<0.05),残余尿量、导尿次数则显著低于组内治疗前(P<0.05),差异均有统计学意义(P<0.05),且联合组治疗后的残余尿量、膀胱最大压力、日平均尿量、导尿次数均显著优于磁疗组治疗后,差异均有统计学意义(P<0.05)。治疗后,联合组的总有效率为93.75%,显著优于磁疗组的79.17%,差异有统计学意义(P<0.05)。 结论 电针八髎、关元、中极穴联合经颅磁刺激可显著改善脊髓损伤后NLUTD患者的膀胱功能、临床症状和生活质量,并提高其临床疗效。

      Objective To explore any effect on the overall prognosis of persons with neurogenic lower urinary tract dysfunction (NLUTD) of combining transcranial magnetic stimulation with electroacupuncture at the Baliao, Guanyuan and Zhongji acupoints. Methods Ninety-six NLUTD patients were randomly divided into a magnetic therapy group and a combined group, each of 48. Both groups received conventional therapy and transcranial magnetic stimulation, but the combined group also received electroacupuncture stimulation at the Baliao, Guanyuan and Zhongji acupoints. Before and after 8 weeks of the treatments, the number of catheterizations and the average urine volume of both groups were recorded. Residual urine volume and maximum bladder pressure were recorded, and the maximum renal pelvis separation width was measured. Both groups were also assessed using the International Lower Urinary Tract Symptoms and World Health Organization′s Quality of Life Brief Inventory. Results After the treatment there was a significant increase in the maximum bladder pressure and the average daily urine output in both groups, but a significant decrease in the average residual urine volume and the number of catheterizations. The combined group showed significantly superior improvement in all of those measures compared with the magnetic therapy group. Tshe therapy′s total effectiveness rate in the combined group was 93.75%, significantly better than the 79.17% of the magnetic therapy group. Conclusions Combining electroacupuncture at the Baliao, Guanyuan and Zhongji acupoints with transcranial magnetic stimulation can improve the bladder function, clinical symptoms and life quality of NLUTD patients. It is more effective than transcranial magnetic stimulation alone.
