| 目的 本研究旨在利用功能性近红外光谱成像技术(fNIRS)观察重复外周磁刺激(rPMS)与电针刺激廉泉穴对健康受试者吞咽相关皮质血流动力学的即时影响。 方法 采用随机数字表法将75例健康受试者分为对照组、电针组及外周磁刺激组,每组25例。研究期间对照组不给予任何特殊干预,电针组给予单次廉泉穴电针治疗,外周磁刺激组给予单次廉泉穴rPMS治疗。于干预前、干预后通过fNIRS采集各组受试者在执行空吞咽及咀嚼任务时的大脑皮质血流动力学数据,并对目标感兴趣区(ROI)氧合血红蛋白(HBO)的浓度及β值进行组内与组间比较分析。 结果 干预后在执行空吞咽任务时,外周磁刺激组与电针组左侧初级运动皮质区(L-M1)的HBO浓度均较干预前显著升高,且L-M1、左侧前运动皮质区(L-PM)及辅助运动皮质区(SMA)的β值亦显著提升;在执行咀嚼任务时,仅发现外周磁刺激组L-M1和右侧初级运动皮质区(R-M1)的HBO浓度显著升高,且L-M1脑区的β值亦显著提升。通过组间对比发现,干预后在执行空吞咽任务时,电针组多个吞咽相关脑区的激活幅度及β值均较对照组显著提升,外周磁刺激组L-M1脑区的激活幅度亦较对照组明显增强,且多个吞咽相关脑区的β值显著提升;此时外周磁刺激组与电针组各脑区的激活水平及β值组间差异均无统计学意义(P>0.05)。 结论 单次rPMS或电针刺激廉泉穴均可对健康受试者的吞咽皮质功能产生调控效应,且作用效果相当。 |

| Objective To observe any immediate effect of repeated peripheral magnetic stimulation (rPMS) and electroacupuncture (EA) at the Lianquan point (CV23) on swallowing-related hemodynamics in the cerebral cortex. Methods Seventy-five healthy subjects were randomly assigned to a control group, an EA group or an rPMS group, each of 25. The EA group received a single session of EA at the Lianquan point, while the rPMS group received a single session of rPMS. There was no specific intervention for the control group. Before and immediately after the interventions, the cortical hemodynamic signals during empty swallowing and chewing tasks were collected using functional near-infrared spectroscopy. Oxyhemoglobin (HbO) concentrations and task-related beta (β) values were also analyzed. Results Intra-group comparisons showed that during empty swallowing, HbO in the left primary motor cortex (LM1) increased significantly on average after either intervention, with corresponding rises in β-values and in LM1, LPM, and supplementary motor area (SMA). During chewing, however, only the rPMS group showed increased HbO in its bilateral M1 areas and a higher β-value in LM1. During empty swallowing, the EA group exhibited greater activation amplitude and higher β-values in multiple swallowing-related regions than the control group. The rPMS group also showed stronger LM1 activation and elevated β-values in several areas compared to the controls. There were no significant differences between the rPMS and EA groups. Conclusions A single session of either rPMS or EA at the Lianquan (CV23) acupoint can modulate swallowing-related cortical functioning in healthy subjects, with comparable effects between the two interventions. |
