      目的研究脑性瘫痪(CP)患儿的钙、磷、骨代谢特点及其与相关调节因素的关系。 方法测定CP组(CP患儿40例)与对照组(健康儿30名)的血钙(Ca)、磷(P)、碱性磷酸酶(ALP)、甲状旁腺素(PTH)、降钙素(CT)、骨钙素(BGP)及桡骨骨密度(BMD),24 h食物Ca、P摄入量及尿、便的Ca、P排出量。 结果2组血Ca、P差异无显著性(P>0.05);ALP、BGP值CP组增高,与对照组比较,差异有显著性(P<0.01),且2者升高呈显著正相关(r=0.6519,P<0.01);PTH、CT值,重度CP患儿与对照组比较,差异有显著性(P<0.01);BMD值,2组间差异有显著性(P<0.05);CP组尿Ca量增高,并与PTH、CT分别呈正相关和负相关(r=0.8512,P<0.01;r=-0.7092,P<0.01),重度CP患儿Ca、P代谢平衡为负平衡,与对照组比较,差异有显著性(P<0.05)。 结论CP患儿存在Ca、P、骨代谢紊乱;增大CP患儿运动量、减少固定及增加Ca、P摄入量可促进Ca、P沉积于骨,提高BMD,预防骨质疏松所致的骨折。

      Objective To study the metabolization of calcium (Ca) and phosphorus (P) and its regulation in children with cerebral palsy (CP). MethodsForty CP children were enrolled in this study and 30 normal children served as control. Plasma levels of Ca, P, ALP, PTH, CT, BGP, BMD and Ca, P in food, urine and stool in 24h were analyzed. ResultsThere was no significant difference in the serum levels of Ca, P, and BMD between the CP children and the control (P>0.05). The serum levels of ALP and BGP in the CP children were significantly higher than those in the control (P<0.01), and there was a positive correlation between the serum ALP and BGP levels (r=0.6519,P<0.01), there were significant difference between the CP children and the control in regard to serum PTH, CT and BMD levels (P<0.01) .The levels of urine Ca of the CP children were increased, and had positive and negative correlations with the serum PTH and CT, respectively (r=0.8512, P<0.01; and r=-0.7092, P<0.01). A negative balance of Ca and P metabolization was demonstrated in the severe CP children as compared with that of the controls (P<0.05). ConclusionThe results showed that disorded Ca, P and bone metabolization can be found in the CP children.
